Hi! My name is Amy Martin, BSHS, and I am a Service Facilitator in Sauk County at Seasons Counseling. I hold a Bachelor's degree in Science and Human Services from the University of Oshkosh. With 15 years of experience in case management, I have been dedicated to supporting individuals in self-advocacy, addressing long-term care needs, and fostering active engagement within their homes and communities. I take great pride in collaborating with both consumers and providers to develop creative solutions for mental health challenges. Outside of work, I enjoy spending quality time with my family and being outdoors.
